Partner Service Agreement

If you are a healthcare provider, advocate, or authorized representative (“Partner”) purchasing appeal credits from Claimable, Inc. (“Claimable”), the following terms apply:

Credits: Appeal credits allow Partners to sponsor appeals for patients, making the appeals free to patients. Credits are purchased in appeal packs or bundles (e.g., 10-pack, 30-pack, or 100-pack of appeals). Each credit represents one submitted appeal.

Credit Usage:

  • When a Partner refers a patient to Claimable to start an appeal, a credit is provisionally transferred to the patient.
  • If the patient submits the appeal, the credit is spent.
  • If the patient does not submit the appeal and takes no action within 30 days, the credit is returned to the Provider’s account for reuse.

Responsibility: Providers agree to accurately input patient information when sponsoring appeals and affirm that they have obtained any necessary consent or authorization to refer patients for appeal services.

Commercial Relationship: These terms form the Order Form referenced in Claimable’s Partner Terms of Service ("Terms") and Business Associate Agreement ("BAA"). The purchase, management, and use of credits are governed by both these Terms and the BAA.

No Refunds: Credits purchased are non-refundable except as required by law.
